I'm having a similar problem with Through Fire and Flames.

I've completed the investigation having met Drakan, and taken the radical path. 

Mission ended with a long delay after teleport. 

-character gone, screen fixed on his last position, unable to bring up menus or do anything other than pivot screen around characters last position 

After a couple minutes, the loading screen popped up and I was returned to the ship without going to the rewards screen.

Investigation is still open at 6/7 without the option to progress it. I've reloaded the game, completed a minor mission on the same planet, and started a priority investigation to no avail.
