When using the concentrated salvo heavy bolter skill with a controller (RT button, no directional input) the shots go straight up and miss the red hilighted targets. Edit: This happens mostly on hive-world maps.

This was happening for me last season.  I mained a heavy bolter and rarely used that skill because they would never hit anything while using a controller.
