Is this a bug or do they really expect somebody to use the inoculator 20,000 times? Because man, that is a lot of times.

This does not require you to use it 20.000 times but to heal that many with the assistance of an Inoculator. Our internal tests recently showed that the counter does not work correctly - like if you heal 3000 it will recognize only a portion (100+). We fixed this on our end and will patch the game soon!
